How to find out if someone who passed away had a life insurance policy in Spain
-
Request the Modelo 790 form on the Ministerio de Justicia website or at any local delegation.
-
Fill out the document and pay the corresponding fee.
-
At least 15 days after the date of death, go to the Registro General de Actos de Última Voluntad (General Registry of Last Wills).
-
Within 7 days you'll receive a certificate listing all life insurance contracts active under the deceased person's name — along with the insurers who issued them.
If the deceased was a Life5 customer, you can also contact us directly at ayuda@life5.com or +34 911 984 986 and we'll help you in English.

Steps to file a claim with Life5
1. GATHER THE DOCUMENTATION. See the required documents for each case below.
2. SEND US THE DOCUMENTATION. Via email, through the customer portal, or book a call and we'll help you. Have the policyholder's NIF/DNI ready so we can identify the policy fast.
3. WE PROCESS YOUR CASE. Once we have the documentation, AXA (our underwriter) must pay the claim within 40 days per Spanish insurance law (Ley 50/1980 Art. 20). In practice, cleanly documented claims are typically paid within 2–4 weeks. Our English-speaking team handles all translation and submission to AXA on your family's behalf — no Spanish required from you.

Modify your policy
We understand your life changes, and with it your needs. That's why we offer the flexibility to modify your policy easily and fast — all in English.
How to make changes from your customer area:
- Log in to your account.
- Select the modify option.
- Make the required changes: update your bank account, adjust coverage, change beneficiaries, update personal data, etc.
Most changes apply instantly. Changes that affect the premium (adding riders, raising insured capital) require a brief AXA re-quote and your acceptance. If you need help, book a call with one of our English-speaking advisors.
